Students Applying from Abroad
In order to issue an I-20 we require the following documents:
Filled application form from Midwestern Career College
Copy of passport (Valid at least six months)
Copy of high school diploma (Intermediate certificate)
An original Affidavit of Support on stamped paper or Form I-134
An original Bank Statement ($26,000.00 minimum)
TOEFL (if required by the U.S. Consulate)
Application fee: $200.00 (non-refundable)
Note: Student has to pay SEVIS fees of $200.00 at www.fmjfee.com
