Change of Status Students
In order to issue an I-20 with the request of change of status we require the following documents:

Filled application form from Midwestern Career College

An explanation letter

Copy of passport with I-94 (valid)

Copy of approvals if any

Copy of credentials or high school diploma (Intermediate certificate)

An original Form I-134 (Affidavit of Support) or an affidavit of support on stamped paper

An original Bank Statement ($20,000.00 minimum)

Application fees $200.00 (non-refundable)
